Foreign Pooled Investment Vehicle: Does not apply unless the LLC was formed outside of the US and is registered to business in the state.
Reporting Company Legal Name/Alternate Trade Name: Your LLC's name is the reporting company. Trade name only applies if your LLC has a DBA/ABN.
Tax ID Type: You may use your social security number or an EIN if you requested one for your LLC. (Request us to file an EIN for you: https://www.montanacorporate.com/llcservices)
Jurisdiction of Formation/first registration: United States, Montana (Tribal jurisdiction does not apply.)
*The exception to this is if you formed your company outside of Montana and qualified it as a foreign LLC in Montana. This does not apply to most Montana LLCs.
Address: Your LLC's address (55 W 14th St Suite 101 Helena, Montana 59601).
Existing Reporting Company: Check only if your LLC was formed prior to January 1, 2024. Refer to your Articles of Organization for the formation date of your LLC. If your LLC was formed prior to January 1, 2024, you may click “Next” to proceed to Part III of the
report.
If your LLC was formed on or after January 1, 2024, you must complete the rest of the information before proceeding. Complete the information on this page with YOUR personal name and home address. You will be required to upload a copy of a driver's license, state ID, US passport, or foreign passport. Once
you have completed the rest of this page, click “Next” to proceed.

Fields for Lines 35, 36, 37: Optional, only complete these lines if they apply to your LLC.
Legal Name/Date of Birth/Residential Address: YOUR personal name, birthdate, address.
Form of ID: You must enter your ID info and upload your ID, even if you uploaded it on the previous page. Once you have uploaded your ID and completed the prior fields, you may click “Next” to proceed.
Final Page: Submit
Complete this page with your personal email, first and last name, and check the boxes to agree that you have completed the form with true and correct information and that you are indeed human. Click “Submit” and DO NOT close the page until you receive a Submission Status Confirmation. You may download the transcript of your report if you wish, and can close the window at this time- You are done!
